Description of the Tattoo Service:

The client agrees to receive a tattoo located on the left forearm, approximately 4 inches in length, using the design provided by the tattoo artist. The procedure shall be performed professionally and in a sanitary environment.

Risks and Acknowledgment:

The client acknowledges that tattooing involves certain risks, including allergic reactions, infections, and skin scarring. The client confirms having been informed of these risks and agrees to proceed voluntarily.

Consent:

The client grants permission for the tattoo artist to perform the procedure described and affirms that all information provided is accurate and complete. The client understands the aftercare instructions and agrees to follow them to promote healing.
