Health Coaching Agreement Sample

Description of Services:

The health coach agrees to provide personalized health and wellness coaching to the client, including strategies for nutrition, exercise, lifestyle modifications, and accountability sessions as detailed in this agreement.

Coaching Sessions and Fees:

Sessions shall be scheduled as mutually agreed upon, with a duration of approximately 60 minutes each. The client agrees to pay a fee of $150 per session, payable prior to each session via electronic transfer or other agreed upon methods.
